Nearly a year in — from token launch to live mainnet transactions, peer-reviewed science, and a growing clinical network.
Nearly a year into our journey, SpineDAO has moved from token launch to a live, production-grade tokenized clinical data platform — with real $SPINE flowing to real surgeon wallets on Solana mainnet. This is the honest Q1 2026 account of what we've built, what the science looks like, and where we're going.
WHERE IT STARTED
$SPINE launched on Bio Protocol V1 in May 2025 and raised $827,500. The mission hasn't changed: build the intelligence infrastructure for spine care — clinician-owned, data-driven, and governed on-chain.
March 19, 2026 was the first production proof: a spine surgeon uploaded anonymized patient outcome data to SpineBase, and $SPINE tokens were transferred to their Solana wallet automatically. Not a simulation. A live mainnet transaction. That's the loop the whole architecture was built toward.
THREE PLATFORMS, ONE ECOSYSTEM
SpineBase (spinebase.app) is the tokenized data registry. Surgeons contribute anonymized outcomes data and earn $SPINE from a dedicated 99M $SPINE data reward pool. The data feeds AI models. Researchers access it through a staking-gated query API. As of March 2026: 370 patients across two campaigns (SI fusion + TLIF), an on-chain data contribution smart contract deployed, and a 3-arm TLIF outcome prediction model live at spinebase.app/predict.
Spinal (spinal.science) is the clinical network layer — a private, verified platform for spine surgeons. Case discussion, micropublications, expert polls, AI-assisted research via Chronos. 200+ verified surgeons. 2,066 clinical vignettes reviewed. 210,000 $SPINE distributed. Recent additions: 5-tier clinician verification, PWA push notifications (iOS + Android), multi-image clinical case posts, interactive expert polls, and $SPINE rewards for clinical decision-making participation.
Spine Reviews — the structured expert crowdsourcing engine inside Spinal — is now heading to the podium.
SCIENCE MILESTONE: IMAST 2026
SpineDAO's first peer-reviewed scientific presentation will be delivered at IMAST 2026 (International Meeting on Advanced Spine Techniques).
Dr. Virginie Lafage will present: "Spine Reviews: Crowdsourcing Global Spine Experts Knowledge via Digital Ledger Technology" — AI & Machine Learning · Novel Technique. Authors: Bassel, Guillaume, Nassim, Joseph, Vincent, Virginie — On Behalf of SpineDAO.
The study: 52 spine specialists across 8 countries reviewed 500 standardized low back pain vignettes on a Solana-secured platform. 2,066 reviews completed at a cost of $0.97 per review. Agreement hit 97.2% consensus for red flag/emergency cases. The dataset trained an AI treatment prediction model.
This is the first published demonstration that decentralized ledger technology can aggregate global expert clinical judgment at scale — rapidly, cost-effectively, and with cryptographic credentialing.
THE TLIF-BAYES STUDY
In parallel, we launched TLIF-BAYES — a Bayesian hierarchical multicenter study comparing Open, MIS, and Endoscopic TLIF outcomes, led by Dr. Anton Denisov, MD. Thirteen contributing surgeons confirmed. Statistical framework: R + Stan (brms), with informative priors from a 2026 meta-analysis. Primary endpoint: revision surgery causality, 9-code taxonomy.
Current registry: 245 patients. Preliminary results targeting the NSpine Southern European Summit — Bastia, Corsica, May 14–16, 2026.

Q2 2026 ROADMAP
The next milestone is Bastia — May 14–16. A surgeon uploads outcomes data on stage. $SPINE arrives in their Phantom wallet. The room sees Solscan confirm it. That's the moment that turns skeptical clinicians into contributors.
Beyond Bastia: TLIF enrollment to 400+, Anton's first Bayesian model run, the Spine Reviews TLIF campaign post-conference, and the rewards distribution smart contract moving to audit.
